Community palliative care

Our Winchester Hospice community palliative care team enables patients to access specialist palliative care as well as end of life care in their own homes.

Brought into the home is the expertise and knowledge of our palliative care specialist nurses, doctors, psychologists and carers, who are there for patients with life-limiting illness and those who require care at the end of their lives.

At Winchester Hospice, we provide community care for patients seen at the Winchester city surgeries of St Clements and St Pauls as well as Friarsgate Practice.

Specialist palliative care

Our specialist palliative care team are there to provide high quality, compassionate care for patients with life-limiting illness in their own homes, supporting them to live a life as full as possible. This includes symptom control, social support, psychological support as well as advanced care planning. What we mean by advanced care planning is the concept of our specialist staff empowering our patients to plan their future care. This involves the creation of a plan that all healthcare professionals can use to inform decisions about medical care- a plan that could include care at home preferences as well as end of life wishes.

Winchester Hospice’s community services are there to ensure that patients with life-limiting illness can be supported when they need it, but also where they need or would like it.


End of life care

Our homecare team supports patients in their own homes who are in the final few weeks and days of their lives. Our wonderful team of five carers provide personal care to patients and work closely with district nurses for nursing requirements. Our homecare team work within the community palliative care team service and feedback any concerns or requirements to the community palliative care team of clinical nurse specialists and doctors.

Our team always support patient wishes to die at home, if that is what is preferred.

Community outpatient services

Our outpatient clinics enable us to see patients for symptom control, advanced care planning as well as psychological support from our in-house expert psychologist.

Good to know

All of our community palliative care staff have access to Winchester Hospice bereavement support and are able to sign post loved ones and families to local bereavement services.

Our Winchester Hospice community palliative care team works closely with local GPs, district nurses, overnight nursing services, NHS and private hospital services as well as other local hospices and charities.

Service opening times

We provide community palliative care Monday – Friday 8:30-16:30.
